As a Patient of Internal Medicine & Pediatrics Associates of Tallahassee, (impacthealth) you have certain Rights
  1. YOU have the right to be treated Fairly and with Dignity.
  2. YOU have the right to be greeted by kind and courteous staff at impacthealth.
  3. YOU have the right to receive information about your medical condition in a concise fashion and in terms you can understand.
  4. YOU have the right to have all of your questions answered by our Staff and our Doctors.
  5. YOU have the right to expect privacy and confidentiality concerning your medical care.
  6. YOU have the right to be informed of all of your options and to consent to or refuse our care.
  7. YOU have the right to participate in decisions concerning your care.
  8. YOU have the right to ask any questions about our services related to cost.
  9. YOU have the right to be treated fairly and without discrimination. It is the policy of Internal Medicine & Pediatrics Associates to treat all patients regardless of age, race, color, handicap, ethnicity, or religious beliefs.

The Responsibilities of the Patient

  1. YOU must be kind and courteous to our Staff and other patients.
  2. YOU have a responsibility to keep your appointments and to be on time. If you cannot keep your scheduled visit, please call us at once and reschedule or cancel so another patient may use the visit.
  3. YOU have the responsibility to provide impacthealth with accurate medical information so that we can make an accurate diagnosis and provide appropriate care.
  4. YOU have the responsibility to follow the course of treatment outlined by your physician and notify us if any changes in your condition occur.
  5. YOU have the responsibility to pay your bill promptly, and to provide our office with up to date insurance information. It is important to notify us of any changes in your insurance status.
