Your First Appointment
On your first visit, we will begin a permanent record. A registration form will be completed at this visit. Information required will include past medical and surgical histories, allergies to medications, and a complete list of your current medications with dosages.
Please bring you insurance card with you. If there is co-pay with your insurance, it is expected at the time of your registration.

We know that you want to be seen at the time of appointment and we try to avoid delays. However, if an emergency arises or if we are delayed, we will make every effort to notify you. We ask for you understanding.

Doctor /Patient Relationships
The physician-patient relationship is one of mutual understanding, trust and respect. As your physicians we will advocate for you as you navigate through the medical system. Together, we will make consensus decisions about your well-being. We will make a special effort to explain everything to you regarding your condition, medicines, treatment options, etc. Please do not hesitate to ask any questions that may arise during the course of your care.

Prescription Refills
Prescription refills, including pain medicine, will be handled Monday through Friday 9:00 am to 4:00 pm when the Doctor is in the office and has access to your medical records. Refills should be requested before you supplies are exhausted.

Laboratory and X-ray results
If necessary your doctor may recommend lab studies or x-rays. Our office usually receives the results in approximately five working days. We ask that you call and inquire about your results after one week. Because of our work routine, normal results may be given to you by one of our staff members. You may always ask to speak with you Doctor about the results of your tests.

Financial Policy
Your medical insurance is a contract between you and your insurance company. The responsibilities of each party are clearly spelt out in your insurance policy. However, as a courtesy to you, our office will bill your insurance company for your medical expenses. If you are not covered by Medicaid, Medicare or an HMO, we request that you make alternative arrangements for payment of your medical bill on the day that you are seen in our office. If you do not have medical insurance, payment must be made in full at the time of treatment, unless other arrangements have been made in advance. For your convenience payments for medical expenses can be made in cash, by check or credit card. If payment of your balance causes financial difficulties please speak to our billing office regarding alternative payment arrangements. Our billing office is open Monday through Friday 8:00 am to 4:00 pm. Please feel free to call us regarding questions about your bill.

Confidentiality
We assure you that your medical records will be handled with the strictest confidentiality. We will not give out information regarding your condition to your employer, friends, relatives, or insurance company without your expressed written permission.
